Our 2024 EOY Showcase is a community oriented event where our music students can showcase their hard work and dedication live on-stage. This homely occasion is an opportunity to come together, celebrate music, and build relationships.

We are excited for your performance! Members of our team will be there to support you. Your audience is ready to and clap and cheer for you!

I understand that i) if – for any reason – I or my child (the registrant) withdraws, does not perform, is absent or late at the time they are called to the stage, is not equipped to perform, or is unprepared to perform they will be exempt from performance and there are no refunds of associated ticket/s fees, ii) for a registrant to be considered equipped and prepared to perform those playing hand held instruments must bring their own instruments, those requiring sheet music or lyrics sheets must bring their own copies, and those requiring instrumental/electronic accompaniment must submit them in email at least 24hrs in advance of show date, iii) in the unlikely circumstance of the event being rescheduled, associated credits will be issued to ticket holders, iv) performance time preference are taken into consideration however, show and time allocations by administration are final, and v) upon submitting this form, registration is final and cannot be canceled.

Consent & Release *

I – the registrant, OR the parent or legal guardian of the registrant – hereby grant permission to Melodique representatives, to take and use video, photographs and/or digital images of myself of my child taken during the EOY Showcase event for use in: 1. Media releases, media articles – including newspapers, radio, and television – printed publications and/or educational materials relevant to the Melodique, 2. Electronic publications and communications such as Melodique’s Facebook site and website. 3. I agree that my or my child’s name and identity may be revealed in descriptive text or commentary in connection with the video/image/s if they/I are/am or have been a student at Melodique. 4. I authorise the use of this video and images without compensation to me. All negatives, prints, digital reproductions shall be the property of Melodique.
